Abstract 15652: Patient Perceptions and Familiarity With Medical Therapy for Heart Failure

  • IntroductionThere are significant gaps in the use of guideline-directed medical therapies (GDMT) for heart failure (HF). The degree to which patient perception and familiarity with GDMT contributes to low utilization in US clinical practice is unclear.MethodsStudy participants were recruited from the GfK KnowledgePanel®, a probability-sampled online panel designed to be representative of the US adult population. English-speaking adults who met the following three criteria were eligible1) told by a doctor that they had HF; 2) currently taking medications for HF; 3) no history of left ventricular assist device or cardiac transplantation. Of 30,707 eligible KnowledgePanel members who received the initial survey, 15,091 completed the screening questions, 440 started the full survey, and 429 completed. All response data were weighted and adjusted for the sampling design.ResultsThe 429 participants had median age 68 yrs (IQR 60, 75) and were predominately white (74.6%), male (70.9%), and with at least a high school education (95.3%). The majority of survey responders reported familiarity with beta-blockers (BB), ACE inhibitors (ACEI), angiotensin receptor blockers (ARB), and diuretics. Less than 25% reported familiarity with angiotensin-receptor neprilysin inhibitors (ARNI) or mineralocorticoid receptor antagonists (MRA). Over 40% of patients reported safety concerns regarding ACEI/ARB and >33% reported safety concerns with BB, MRA, ARNI, and diuretics. Between 25%-40% reported concerns with effectiveness of BB, ARNI, MRA, and diuretics, while 40% were concerned over the effectiveness of ACEI/ARB (Figure).ConclusionsMany HF patients are not familiar with therapies for HF, particularly ARNI and MRA. Among patients who are familiar, significant proportions question the effectiveness and safety of therapies. Enhanced patient education and decision support may be effective strategies to improving real-world uptake of GDMT for HF.
