  • Maintenance of water balance is very tightly regulated between the hypothalamus, pituitary gland, and kidneys. The main hormone involved in this system is arginine vasopressin (AVP) or antidiuretic hormone. This chapter discusses the etiology/pathophysiology, history and clinical signs, diagnosis, therapy of polyuria and polydipsia in animals. Causes of primary polyuria can be divided into those which result in an osmotic diuresis versus manifestations of diabetes insipidus (DI) in which there is AVP insufficiency or insensitivity. Loss of appetite and lethargy may raise the possibility of liver or renal disease whereas polyphagia may increase the index of suspicion for hyperthyroidism in cats or hyperadrenocorticism in dogs. Diagnosis involves first‐tier and second‐tier tests as well as modified water deprivation test. Treatment will need to address the underlying disorder. For patients with central DI, desmopressin acetate AVP will need to be administered daily.
