  • This is a unique question‐and‐answer chapter for surgical residents and trainees, concentrating on the infectious disease. Ventilated patients who develop pneumonia suffer from a mortality of up to 46%, and ventilator‐associated pneumonia (VAP) is the second most common health care‐associated infection in the United States. The use of clinical criteria alone, rather than the combined use of clinical criteria and semi‐objective measures such as the Clinical Pulmonary Infection Score (CPIS), is recommended for use to determine the need for initiation or discontinuation of treatment for VAP. Shorter treatment courses reduce antibiotic exposure and recurrent pneumonia due to multidrug resistant (MDR) organisms. Standard treatment of Clostridium difficile infection consists of metronidazole and oral vancomycin up to four times daily. Prophylaxis with fluconazole in non‐neutropenic critically ill patients with risk factors for candidiasis reduces invasive fungal infections and improves mortality.
