Metaplastic carcinomas of the breast are a heterogeneous group of invasive carcinomas that have squamous and/or mesenchymal differentiation. This chapter discusses the clinico‐pathological features of metaplastic carcinomas and current recommended treatments in light of currently available data. Metaplastic carcinomas represent fewer than 5% of breast malignancies. Metaplastic carcinomas are tumors that contain one or more populations of cells that appear to have undergone metaplasia, a change from glandular to nonglandular morphology. High‐grade monophasic tumors have been classified as pure sarcomas and low‐grade spindle cell metaplastic tumors are often misinterpreted as fibromatosis. Participation in clinical trials incorporating targeted agents targeting the PI3K and epithelial‐to‐mesenchymal transition (EMT) pathways are recommended for the treatment of patients with early and advanced metaplastic breast cancers as a whole.
