• Orofacial granulomatosis (OFG) is an uncommon chronic inflammatory disorder limited to the orofacial tissues. Recurrent or persistent labial swelling is the leading feature. Associated gingival hypertrophy is a characteristic diagnostic feature. In the majority of cases, OFG represents a distinct idiopathic entity, but concomitant Crohn's disease should be considered and patients monitored accordingly. In view of the specific oral features, perianal involvement, gastrointestinal symptoms and abnormal faecal calprotectin, underlying Crohn's disease should be suspected and a gastroenterology review requested. The potential role of a hypersensitivity reaction to food additives, toothpastes and dental materials remains unclear. Recommended treatment options for OFG include topical tacrolimus and corticosteroids, intralesional and systemic corticosteroids, antimicrobial agents and, more recently, TNF‐α inhibitors. For severe, recalcitrant cases, surgical debulking treatment may be beneficial.
