Premature atrial and ventricular complexes are disturbances of the cardiac rhythm in which ectopic impulses arise from a muscle fiber or group of fibers within the atria or ventricles producing irregularity in the heart rhythm. Premature atrial contractions (PACs) are the most common arrhythmia seen during ECG monitoring. Although the exact cause of PACs is unclear, they are a premature activation of the atria from ectopic electrical impulses that trigger the atria before depolarization from the sinus node. An extreme form of atrial ectopy is multifocal atrial tachycardia in which multiple ectopic foci are depolarizing rapidly and at irregular intervals. Ventricular ectopy or premature ventricular contractions (PVCs) result from abnormal electrical discharges from Purkinje fibers in the ventricles rather than the sinoatrial node. These electrical discharges produce a premature heartbeat and irregularity in the heart rhythm. PVCs are frequently seen in clinical practice, typically benign, and often do not require treatment.
