  • Melanoma incidence and mortality rates continue to rise worldwide, driven by increased ultraviolet light exposure (both natural and artificial sources) and differences in prognosis according to age, anatomic location of the primary, and sex. Despite significant recent progress in the treatment of patients with advanced disease, early detection of thinner cutaneous melanoma remains the best chance for cure. Advances in genomics have allowed a more nuanced understanding of melanoma, moving beyond the traditional dysplastic nevus‐to‐melanoma model and identifying multiple divergent oncogenic pathways that lead to tumor formation. Knowledge of the molecular mechanisms driving melanoma opened the doors for the development of targeted therapeutic approaches, as awareness of immune checkpoint blockade has similarly resulted in successful immunotherapy approaches. As we enter the era of personalized medicine, improved understanding of the molecular and immunologic basis of melanoma will enhance risk stratification, screening and prevention efforts, genetic counseling, treatment customization, and survivorship.
