  • A 47-year-old man suffering from epigastralgia was admitted to the hospital. CT showed a pancreatic tumor, 5cm in diameter, located at the tail of pancreas with multiple liver tumors. MRI showed that the pancreas tumor detected low intensity area in T1 and high in T2. AFP and PIVKA-II levels were elevated. He was diagnosed as a pancreas tail carcinoma with multiple liver metastases. We performed a liver tumor biopsy to determine the chemotherapeutic regimen, and those findings revealed hepatocellular carcinoma (HCC). We then performed a laparoscopic pancreas tumor biopsy to clarify diagnosis, which pathological finding revealed metastatic pancreatic carcinoma from HCC. The patient was finally diagnosed with metastatic pancreatic carcinoma due to HCC. Metastatic pancreatic carcinoma is very rare, and frequent sites are kidney, lung and stomach cancer. Here, we report a very rare case of metastatic pancreatic tumor from hepatocellular carcinoma with a review of the literature.
