Achalasia and eating disorders share numerous clinical features. Significant overlap in symptoms and signs exists, most prominently weight loss and vomiting. Untreated, the pain and discomfort associated with achalasia may lead to a severely restricted pattern of eating. Disturbed eating behaviors are reported in individuals with achalasia, such as ritualistic or rule-governed eating, or avoidance of types or textures of food proven difficult to consume. Difficulties with eating and accompanying weight loss may raise suspicions of an eating disorder, especially in young women. Numerous reports have documented cases of achalasia which were erroneously misdiagnosed as an eating disorder. This chapter provides an update and synthesis of current knowledge on diagnostic issues relevant to the differential diagnosis between eating disorders and achalasia, with a focus on the 5th edition of the Diagnostic and Statistical Manual Chapter on Feeding and Eating Disorders.
