Proximal humeral fractures in the elderly are the third most frequent fractures these days, and their incidence is increasing paired with rising osteoporosis’ prevalence and with population aging. Their treatment has been under debate in the last decades, and this chapter aims to resume all the evidence available. Nowadays, the vast majority of displaced proximal humeral fractures in the elderly are treated conservatively, based on current evidence. As we are going to review in the following pages, results for the different surgical options are poorly predictable, and avoiding complications should become the most important objective in this population of frail and elderly patients.
