Colorectal cancer is a very rare disease in children and associated with a high frequency of tumor preposition syndromes. Multimodal treatment resulted in considerable improvement of survival although related with late and long-term risk of toxicity. Oxaliplatin-induced neuropathy is frequently observed and may take years to completely resolve, particularly in the treatment of children. Molecular mechanisms are described as well as potential protective measures. Bowel dysfunction depends on the surgical procedure with long-term data available particularly on ostomy. Late toxicities of radiotherapy need to be considered due to substantial improvement of outcome thru neoadjuvant chemoradiotherapy protocols for the treatment of rectal cancer. All of the above treatment modalities can affect quality of life in colorectal cancer survivors with mental, physical as well as social aspects being addressed in detail. Late effects related to tumor predisposition syndromes include second malignancies thus underlining the necessity of enrolment in surveillance programs. Different screening procedures depend on the underlying genetic disposition and are further described in detail.
