  • The organisms responsible for infective endocarditis are separated from the bloodstream by layers of fibrin and platelets, the elements which compose vegetations. Indeed, studies have shown that antibiotic behavior in fibrin clots is very different from that seen in experimentally produced vegetations in vivo. super(14)C-teicoplanin remains concentrated at the periphery of the vegetation and does not diffuse into the core of the vegetation. super(14)C-ceftriaxone progressively diffuses into the vegetation, but a high concentration gradient persists between the periphery and the core when autoradiography is performed one elimination half-life after the first determination. To a lesser degree, this concentration gradient is observed with super(14)C-penicillin. For these two groups of antibiotics, measurement of the concentration in homogenates by conventional methods gives a false indication of the actual level of activity at the core of the lesion. The third most frequent pattern was a homogeneous diffusion throughout the whole lesion; it was observed with tobramycin, pefloxacin, temafloxacin, sparfloxacin and daptomycin.
