Blood-sugar and plasma-insulin levels were measured in the course of oral glucose tolerance tests in twelve uraemic patients, eight with chronic renal failure and four with acute renal failure, and the results were compared with a group of fourteen age and sex matched control subjects. The glucose intolerance of uraemia was confirmed, the blood-sugar values showing a normal rate of rise but a notably delayed fall. The plasma-insulin showed a similar pattern to the blood-sugar. These results exclude defective insulin production or release as the basis for the glucose intolerance of uraemia. It is suggested that a dialysable antagonist, or antagonists, to insulin accumulate in uræmia.
