A case of staphylococcal endocarditis with perforation of a normal mitral valve resulted in severe congestive heart failure due to mitral insufficiency that could not be controlled by medical management. Surgical closure of a perforation of the anterior leaflet was successful, and the patient was returned to a near-normal state.
The literature on staphylococcic endocarditis and endocarditis on normal valves is reviewed. The frequency of involvement of normal valves in acute bacterial endocarditis is emphasized. Surgical repair of the mitral valve after endocarditis is reviewed.
