Dental, skeletal and soft tissue changes following activator treatment were investigated in 78 boys and girls with Class II Division 1 malocclusion. Forty-seven of the patients were successfully treated with an activator, 31 did not receive treatment. There was a significant decrease of the SNA and ANB angles and of the overjet, a retrusion of the upper and lower lips and increased prominence of the soft tissue pogonion in the treated group but these changes were not found in the untreated group. Analysis of individual treatment results showed variability in the antero-posterior position of the lips, though good occlusion was achieved in all patients. Patients with good skeletal pattern at the start of treatment attained a good lip balance after treatment but those who exhibited an initially high or low mandibular plane angle and a poor chin/lip and nose relationship attained a less favourable lip balance after treatment.
