  • Stroma-free hemoglobin solution has been studied as a temporary red cell substitute. It is usually infused at a 6–8 g% concentration so as to be isooncotic with plasma. As a result of this, its O 2-carrying capacity is significantly reduced as compared to whole blood. Additional loss of O 2 carrying capacity can be prevented by lowering the rate of methemoglobin formation. This study tested the efficacy of the reduced forms of nicotinamide adenine dinucleotide (NADH) and nicotinamide adenine dinucleotide phosphate (NADPH) in minimizing methemoglobin formation in vivo during total exchange transfusion in baboons. Thirteen adult baboons underwent total exchange transfusion to a hematocrit of 2%. Seven animals received the hemoglobin solution with the reduced cofactors (NADH and NADPH). The plasma methemoglobin levels at the end of the exchange was 10% in the animals receiving the hemoglobin solution with the added cofactors, and 19% in the animals receiving the solution without the cofactors. We conclude that the addition of the reduced cofactors significantly lowers the plasma methemoglobin levels.
