• To the Editor: The strategies presented by Galvin and Milstein (Sept. 19 issue) 1 are relevant to the current national dialogue; however, the authors neglected to consider models in which providers are paid on the basis of performance. Contrary to what the authors suggest, many insurers are effectively improving the quality of heath care, while also achieving economic efficiencies, by gradually introducing innovative models for the reimbursement of providers on the basis of the quality of their output.
