A male infant was brought to the emergency department at day 6 of life for lethargy and jaundice and was diagnosed with pneumococcal sepsis and meningitis. Soon after admission, he required mechanical ventilation and dopamine circulatory support. Subsequently, a urinary tract infection with Klebsiella pneumonia developed in the patient. Past medical history was only significant for a child born full-term without perinatal complications. There was no family history of primary immunodeficiency, autoimmunity, multiple miscarriages, early childhood deaths, or consanguinity. Both maternal and paternal grandmothers were HIV positive. Neither of the parents were previously tested for HIV infection, but both were healthy. Physical examination was unremarkable, with no evidence of a rash, lymphadenopathy, or splenomegaly.
