Earlier it was believed that the axons of the central nervous system (CNS) were incapable of regeneration, but later it was demonstrated that transected spinal cord axons could regenerate long distances through peripheral nerve grafts. This suggested that the CNS environment played a key role in limiting regeneration and soon CNS myelin and the glial scar were identified as inhibitors of axonal growth. This chapter describes various pharmacological strategies that have arisen from these findings and the way in which these have been used to encourage axonal regeneration in the injured spinal cord. These include the administration of agents that reverse the inhibitory properties of CNS myelin and the glial scar, the transplantation of cells that create a permissive substrate for axonal growth, and the implantation of artificial scaffolds that have been designed to guide regenerating axons through the injury site.
