Currently available cancer chemotherapy exhibits little, if any, specificity despite the fact that drug targeting is an old idea initially formulated by Ehrlich. However, stimulated by an increased knowledge about the tumour physiology and the disposition of drugs in the body, site-specific drug delivery has now become one of the key issues of cancer therapy. This review aims to examine the strengths and weaknesses of targeted drug delivery using a macromolecule-drug conjugate alone or with other therapeutic modalities and look at areas of cancer therapy where it could be of benefit. Attention is also paid to the problems which have to be overcome for the pharmaceutical development and clinical use of soluble macromolecule-drug conjugates.
