The authors evaluated the pulmonary function tests, diffusing capacity of carbon monoxide, and nitric oxide at pre- and 3-month following the implantation of either the HeartMate II (n = 8), HeartMate III (HMIII; n = 2) (Abbott Vascular), or HeartWare (HeartWare ventricular assist device; n = 7) (Medtronic). To date, there are no comparisons between the 3 devices related to the effect on systolic pulmonary artery pressure, pulmonary capillary wedge pressure, or diffusing capacity of the lungs for carbon monoxide. Fontan physiology has demonstrated that pulseless pulmonary circulation is well tolerated however some studies on porcine models have shown its association with increased pulmonary resistance and pulmonary hypertension.4 It is expected that with reduction in LV pressures, the postcapillary pulmonary hypertension improves, however, it is suggested that combined before and after capillary pulmonary hypertension is associated with vascular remodeling unresponsive to left heart pressure reduction.
