  • Subjects completed a valid self-report Bowel Disease Questionnaire 3 and donated a venous blood sample when they were symptom free and when they experienced a self-reported symptom flare at least 1month later. Psychological status has long been implicated in IBS and neuroendocrine mediators involving the hypothalamic-pituitary-adrenal (HPA) axis have immune-suppressive actions. 6 Increased production of stress hormones may explain decreased T cell responsiveness in IBS-D, although we would expect that this would occur in all patients with IBS during symptom flare. Alternatively, T cells may already be in an exhausted state and therefore less responsive to stimulation in IBS-D only, possibly due to a subclinical infection. 7 In conclusion, our results indicate that circulating THELPER cells are inhibited in IBS-D during symptom flare. [...]in order to accurately characterise immune profiles in IBS, researchers should consistently provide information regarding IBS subtype and symptom profile at the time of tissue sampling.
