• Highlights • CLL/SLL spectrum is the most common lymphoproliferative disorder. • Neurological manifestations of NHL are very uncommon. • CNS direct invasion and paraneoplastic neurological syndromes are rare in CLL/SLL. • We present the first reported case of spinal cord parenchyma involvement by SLL. • It could represent direct tumour invasion, but paraneoplastic myelitis is likely.
