Rehabilitation treatment for vertigo and balance disorders has become an accepted therapy that should be taken into consideration when treating otoneurologic patients. The application of rehabilitation treatment for vertigo is limited to select patients suffering the sequelae of a stabilized peripheral vestibular dysfunction. The basic concept of such a therapy involves stimulation of the development of compensation and adaptation by central mechanisms. The stimulus is the presentation of nonadapted situations to the centers, ie, those that elicit vertigo or instability. In addition to the techniques based on stimulation by the nonadapted stimulus, some authors have developed a technique that is supposed to cure the patient with only one maneuver. This technique is based on the theory of displacement of otolithic debris, which is thought to disturb the cupula (cupulolithiasis) or the lumen (canalithiasis) of the posterior semicircular canal. This technique only deals with positional vertigo, whereas a general rehabilitation treatment program addresses all sequelae of peripheral vestibular dysfunction. Emphasis must be placed on recognizing the correct indications and on adequately applying exercises.
