• To the Editor: Perez-Velez and Marais (July 26 issue) 1 restrict the effect of vaccination with bacille Calmette–Guérin (BCG) to the prevention of disseminated (miliary) disease and tuberculous meningitis. The authors did not take into account major progress that has been achieved in understanding the effect of BCG vaccination. Three high-quality observational studies involving 1346 patients established that BCG vaccination in infancy protects against infection with Mycobacterium tuberculosis as detected by an interferon-γ–release assay (IGRA). 2 – 4 The effectiveness of the vaccine for prevention of infection — between 66% and 76% — was shown to be similar in all three studies.
