• As the author writes this, the Medicare Payment Advisory Commission has just made its recommendations for updates to the payment rates for hospital outpatient services and voted its support for the Centers for Medicare & Medicaid Services (CMS) to launch a value-based purchasing program for ambulatory surgery centers by 2016. The Supreme Court is expected to decide this summer whether the individual mandate in the Affordable Care Act is constitutional. Further, the results of the November election will surely influence their direction, regardless of what the Supreme Court does. There was no uncertainty from former CMS Administrator Donald M. Berwick, MD, at last December's Institute for Healthcare Improvement Forum. The reduction of waste is the quality dimension of their time, Dr. Berwick said. In very large measure, improving care and reducing waste are one and the same thing. The waste of overproduction comes from producing what is unnecessary, when it is unnecessary in unnecessary amounts.
