Tremendous advances have been made in HIV prevention, including preexposure prophylaxis (PrEP) and postexposure prophylaxis (PEP). PrEP is a proactive approach that involves taking a combination of antiretroviral drugs daily or on-demand for anticipated sexual events. PEP, on the other hand, is a retroactive approach that involves initiating ARV drugs within 72 hours of a potential exposure. However, there are still barriers to HIV prevention care, such as medication side effects, cost, pill burden, stigma, knowledge gaps, and long wait times in emergency departments and urgent care centers. Postexposure prophylaxis-in-pocket (PIP) is an HIV prevention modality for individuals with infrequent and unanticipated HIV exposures. PIP provides a full 28-day course of PEP prior to an exposure occurring, allowing individuals to self-initiate medications and seek follow-up care on a less urgent basis. PIP is particularly beneficial for individuals with lower frequencies of high-risk exposures. Baseline tests and vaccinations should be administered before prescribing PIP, and patients should be followed up every 6 months or as needed. PIP is a valuable strategy for reducing barriers to PEP care and empowering individuals in their HIV prevention efforts.
