• Outcomes 1. Demonstrate an understanding of how medications contribute to symptoms at end of life 2. Recognize the pharmacokinetic and pharmacodynamic challenges at end of life 3. Follow a systematic approach to deprescribing This interactive case-based session will focus on medication management for patients receiving hospice care. There are many challenges caring for complex patients receiving multiple medications. It is important to distinguish between symptoms secondary to disease progression and symptoms related to adverse drug reactions. Reducing risk of agitated delirium at end of life includes thoughtful deprescribing. A systematic approach to stopping unnecessary and potentially harmful medications will be discussed, as well as a practical discussion of pharmacokinetics and pharmacodynamics at end of life.
