The effective management of perianal fistulas in patients with Crohn’s disease presents an extremely challenging problem. Crohn’s disease is a rather common disease that in more than 20% patients results in perianal fistula, which take long time to heal and frequently recur, causing physical and psychological burden. The therapeutic tools available, medical and surgical treatment have low quality evidence, providing low healing rates and high recurrence of fistulas. Moreover, there is great morbidity associated with the treatments, as immunosuppression and faecal incontinence. There is space for new approaches and therapies. One of the most promising tools are mesenchymal stem cells. These cells have immunomodulating properties playing a significant role in tissue repair. Several clinical trials in Crohn’s perianal fistulizing disease have reported efficacy and safety.The aim of this narrative review is to present an overview of Crohn’s perianal fistulizing disease, outcomes of current medical and surgical therapies, some insight into mesenchymal stem cell mechanisms of immunomodulation and outcomes of mesenchymal stem cells therapies.
