  • Introduction: The proposed histologic desmoplastic reaction (DR) categorization directly reflects tumor behavior in a modulating stromal environment and could provide valuable prognostic information for colorectal cancer patients, but according to our previous report, the prognostic role of DR in pancreatic ductal carcinoma has got opposite prognostic results. We evaluate total resected specimens to investigate the specific characteristics in pancreatic ductal adenocarcinoma. Methods: A total of 28 pancreatic ductal adenocarcinoma patients who underwent potentially curative surgery for their tumors in our hospital (2009 to 2013) were included. DR was classified as mature, intermediate, or immature on the basis of the existence of keloid-like collagen and myxoid stroma-distinctive histologic products of extracellular matrix remodeling. We evaluated all sections in each patient. Results: With regard to DR, 0, 11 and 17 were classified as mature, intermediate, and immature in the largest cross-sectional slice. Fourteen of 17 immature cases show over 10% immature DR. All of intermediate cases did not show immature DR in other sections. In all immature cases, the largest crosssection had the highest immature DR ratio. We can find mature and intermediate DR around pancreatic parenchyma of tumor, and intermediate and immature DR in the extra-pancreatic tissue side of tumor. Conclusion: We evaluate DR categorization in pancreatic ductal adenocarcinoma. All of intermediate cases do not show immature DR in other sections. On the other hand, in immature cases, we find various expression of DR in the same section. We need to analyze with increasing the number of cases examined in order to establish an original categorization system for the pancreatic cancer.
