• Laboratory studies showed the following results: white blood cell, 12,010/μL; serum amylase, 1,570 U/L; C-reactive protein, 79 mg/L; alanine aminotransferase, 419U/L; aspartate aminotransferase of 249 U/L; gamma-glutamyl transpeptidase of 1,140 U/L; serum total bilirubin of 6.37 mg/dL; and carbohydrate antigen of 19–9 (77.41 U/mL). Figure 1. (a) Image from axial contrast-enhanced computed tomography showed the nodule (white arrow) in the distal common bile duct (CBD) mildly enhanced in the venous phase. (b) Magnetic resonance cholangiopancreatography showed obstruction of distal biliary ducts. (c) Gross appearance: Because of the suspicion of cholangiocarcinoma, we performed the Whipple procedure without evidence of distant metastasis.
