  • Aim: Acceptance and Commitment Therapy (ACT) is regarded as an effective treatment option for patients with chronic pain and comorbid psychiatric disorders. Our prospective naturalistic study aimed to study associations of psychological processes that are addressed by ACT with clinical outcomes. Methods: Sixty patients with chronic pain and psychiatric comorbidities received a six-week ACTbased multidisciplinary treatment in a psychosomatic day clinic in Berlin, Germany. Clinical outcomes: somatic symptoms (PHQ-15), depression (PHQ-9, MADRS) and general anxiety (GAD-7); process-specific measures: pain acceptance (CPAQ), mindfulness (FMI) and psychological flexibility (AAQ-1I). Multiple regression and correlation analyses were used to analyse pre- to post-treatment changes of clinical and process-related measures. Results: Changes in CPAQ, FMI and AAQ-II were significantly correlated with changes in PHQ-15 (r=-0.53, -0.52, -0.55, adjusted R2 = 0.42, all p's< 0.001) and PHQ-9 (r = -0.47, -0.40, -0.59, adjusted R2 = 0.38, p's < 0.01 or < 0.001). Significant correlations were also found for FMI and MADRS (r = -0.40, p<.01), as well as for AAQ-II and GAD-7 (r= -0.51, p<.001). Conclusion: Results indicate that changes in processes such as pain acceptance, mindfulness, and psychological flexibility are associated with clinically relevant changes in somatic symptoms, depression and general anxiety. Preliminary data of this ongoing study are presented, while future investigations based on larger samples as well as on long-term outcomes after three and six months will be discussed.
