Tuberculosis presents a global threat in both developing and developed countries. Genitourinary tuberculosis is a form of extrapulmonary tuberculosis that occurs in kidney, ureters, seminal vesicles, prostate, testis, vas deferens, and epididymis. Here, we discuss a case of tuberculosis testis mimicking as carcinoma testis in a 36-year-old male patient who presented with testicular swelling with FNAC report showing germ cell tumor, normal tumor markers, and an equivocal PET scan. A high inguinal orchidectomy revealed necrotizing granulomatous epididymo-orchitis favoring tuberculosis, which was confirmed by PCR. The rarity of this presentation makes the findings important to report.
