• Interprofessional working as "substitution" is strongly advocated in the NHS Plan.1 It flows from the commitment to redesign the health service around the needs and concerns of patients, to end the "demarcation lines," especially those between doctors and other health professionals. According to the secretary of state's introduction to the plan, "For the first time, nurses and other health professionals will be given the bigger roles that their qualifications and expertise deserve.
