Since Alain Cribier's first procedure in 2012 (3), considerable progress has been made with respect to procedural success and 30-day mortality rates. [...]we can control 3 key factors: 1) selection of the transfemoral route whenever feasible and safe; 2) prevention of the occurrence of paravalvular leak >1 grade by optimal valve sizing and positioning and its correction by adequate post-dilation (19), a problem that newer-generation valves will probably solve; and 3) consideration of systematic PM implantation, or even resynchronization, in the presence of an LBBB with large QRS width (>160 ms) for patients with significantly impaired left ventricular function.
