  • The I/D polymorphism of the ACE gene exerts a codominat effect on the plasma ACE level. Diseases known or suspected to be affected by elevated ACE activity would be more likely in patients (pts) with the D allele of the ACE gene. However, the increased ACE activity has never been demonstrated to be a cardiovascular risk factor despite the multiple interactions of ACE with hypertension and atherosclerosis. The role of the I/D polymorphism and ACE plasma activity in hypertension, coronary artery disease (CAD) and restenosis after coronary stenting was evaluated in a consecutive series pts of coronary interventions. Plasma ACE level was measured at admission and quantitative coronary analysis of the coronary arteries was performed before and after the stenting procedure, and at six-month angiography. A total of 730 pts were studied, 333 hypertensive (47%), their ACE level compared to non-hypertensive pts was: 21 ± 15 vs 17 ± 15 U/l (p = 0.014). ACE in 455 pts with single vessel CAD was 19 ± 15 vs 18 ± 15 in 275 pts with multivessel CAD (p = 0.17), and in 158 restenotic pts was 23 ± 20 vs 18 ± 13 in 572 non-restenotic pts (p = 0.0001). Neointimal growth that after injury caused by coronary stenting, depends on plasma ACE activity; and the I/D polymoprhism of the ACE gene is an indirect marker of this process.
