  • In post-menopausal state the oestrogen deficiency is a trigger for the increase of blood pressure and for the unfavorable changes in lipid profile and glucose methabolism. Increased angiotensin converting enzyme (ACE) plasma level has been related to coronary artery disease (CAD) also in post-menopausal women. Activity of the renin-angiotensin system (RAS) is in part regulated by the A/C polymorphism of the angiotensin II type 1 receptor gene (Ang-IIT1R). The role of conventional risk factors, ACE plasma level and the A/C polymorphism of the Ang-IIT1R was studied in 141 pts with CAD with or without in stent-restenosis (ISR) and in a control group of 68 women with a normal coronary angiogram. (See Table) Normal (68) Non restenosis (112) Restenosis (29) p Age (years) 63 ± 10 67 ± 6 64 ± 8 Ns Hypertension 31%°* 66%° 55%* <.001°* Diabetes 4%°* 17%° 25%* <.01°* Tot. Chol. The distribution of the A/C polymorphism of the Ang-IIT1R was similar in hypertensive and non-hypertensive women: χ2 = 0.9, p = 0.6. In post-menopausal women hypertension and other conventional risk factors are associated with CAD but not with ISR. Elevated ACE level is a risk factor for ISR. The A/C polymorphism of the Ang-IIT1R did not identify a group at a higher genetic risk for hypertension, CAD or ISR.
