Objective To study the relation between IGFBP-1 concentration in cervical secretion and preterm labor or fetal infection in lung and to screen a ideal marker of preterm labor. Material and Method Cervical secretion and bacterial vaginosis was sampled in all Sixty-four women, of Sixty-four consecutive women, thirty-four women also were detected serum IL-6, CRP and WBC count. The pregnancy and neonatal outcomes of Sixty-four consecutive women were follow up.Result IGFBP-1 concentration in cervical secretion of 64 samples has a positive relationship to preterm labor and infant infection in lung , but once detection did not predict the detection-to-delivery time. If 21.6215 mu g/ml was used as standardized score, the sensitivity, specificity were best, and the sensitivity, specificity, the positive and negative predictive value were 73.3%, 95.9%, 84.6% and 92.2% respectively. Conclusions IGFBP-1 concentration in cervical secretion is clinically useful for the prediction of preterm delivery among pregnancy women an
