• A case of a young male admitted with diffuse alveolar hemorrhage after recently starting smoking and sniffing glue is presented. He has new onset acute kidney injury and is diagnosed with Goodpasture syndrome after his anti-glomerular basement membrane antibodies return positive. Goodpasture syndrome is a rare pulmonary-renal syndrome where antibodies are directed against the glomerular and alveolar basement membranes. Early recognition is important in order to initiate appropriate treatment with immunosuppressive therapy and plasmapheresis.
