  • Purpose: The purpose of this study is to determine the specific findings in shunt study by transesophageal echocardiography (TEE) in definite paradoxical embolism with deep vein thrombosis (DVT). Method: We reviewed consecutive 149 ischemic stroke patients who received shunt study with TEE to look for right to left shunt (RLS). We classified patients to four categories. Color group (n=18) was defined as the patients who had RLS detected by color Doppler flow imaging. Contrast group (n=64) had RLS detected by contrast injection. In Non significant group (n=24), the amount of contrast shunting was so small to determine as significant RLS. In Negative group (n=43), there was no RLS sign. Results: By compression ultrasonography, DVT was detected more frequently in Color group (6/17, 35%) than in Contrast (2/51, 4%), Non significant (2/14, 14%) and Negative groups (1/3, 33%). In definite seven cases of paradoxical embolism with DVT, Color group was five patients (71%). Conclusion: In almost cases of definite paradoxical embolism with DVT, RLS could be found by TEE with color flow imaging. A part of RLS detected by contrast injection method might be not associated with paradoxical embolism.
