Evidence-based psychotherapies such as family-based treatment (FBT), cognitive behavioral therapy (CBT), and dialectical behavioral therapy (DBT) are often viewed as the treatment methods of choice for individuals with eating disorders. However, they have been primarily studied among patients in smaller bodies and as they are commonly delivered, these treatment methods reinforce weight stigma. Many of the components of these evidence-based psychotherapies perpetuate anti-fat bias and are harmful to fat people. These treatment models are then taught to new clinicians entering the field, perpetuating the cycle of harm. When the treatments most often recommended for people with eating disorders are in fact harmful to a large portion of that group, it is imperative that we consider the treatments themselves and decide if they can be changed to be weight inclusive—or if we need to abandon them altogether. This chapter will evaluate these treatments.
