Carotid body tumors (CBTs), also referred to as chemodectomas or carotid body paragangliomas, are the most common head and neck paragangliomas. Adequate preoperative imaging must define the extent of the tumor, the presence of any synchronous ipsilateral or contralateral tumors, and any associated carotid artery occlusive disease. If the need for distal internal carotid artery (ICA) exposure for tumors extending cranially is anticipated by preoperative imaging, nasal intubation and mandibular subluxation should be strongly considered. Though they are usually benign and indolent, once identified in acceptable-risk patients, tumor resection is indicated. If allowed to grow large enough to cause symptoms, such as a palpable mass or cranial nerve dysfunction, because of its significant vascularity and adherence to neurovascular structures, a CBT will present significantly greater technical challenges during surgical resection. Several imaging studies are available to confirm the diagnosis and provide adequate information for surgical planning.
