  • Type 2 diabetes mellitus (T2DM) is a progressive disease, often requiring exogenous insulin therapy and treatment intensification. Despite new therapies, most patients do not reach the recommended [HbA.sub.1c] target of <7%. This 6-month longitudinal study recruited Croatian patients with T2DM who did not achieve glycemic control with premixed insulin. Patients received once-daily bedtime glargine plus preprandial short-acting insulin analogues under investigators' discretion. Endpoints included percentage of patients with [HbA.sub.1c] <7% at study end and changes in mean [HbA.sub.1c] and fasting plasma glucose (FPG). The intent-to-treat population comprised 220 patients. At study end (6 months), 88 (40%) patients reached target [HbA.sub.1c] <7%, with a mean HbAlc decrease from 8.9±1.4% to 7.3±0.9%. The mean FPG decreased from 11.6±3.2 mmol/L to 7.4±2.3 mmol/L (P<0.0001). Glycemic regulation with premixed insulin in these patients was suboptimal, but intensified basal-bolus treatment using glargine as basal insulin led to significant improvements in clinical control.
