Focused stimulation using implanted devices has so far been limited to highly invasive, intracranial implants using subdural and/or depth electrodes to apply electrical stimulation. A new device is reported which performs electrical focal cortex stimulation transcranially via a 5-channel Laplace-like electrode placed subgaleally over the individual focal region. This device (EASEE) is able to perform low-frequency and high-frequency AC stimulation and DC-like stimulation (DLS). Programming is flexible and also allows for patient-triggered ictal stimulations.
In two clinical trials, combined AC and DLS over a period of 6months in a total of 32 participants resulted in a significant reduction in seizure frequency with a responder rate of 53%. Safety was excellent with local skin irritation at the implantation site as the only typical implantation-related adverse effect.
