Abstract Background Acinic cell carcinoma (ACC) of the parotid gland is a rare salivary gland malignancy with a low propensity for perineural invasion. Here we present a case of delayed recurrence presenting as facial nerve paralysis with imaging and pathology demonstrating tumor erosion of the Fallopian canal. Methods Case report with clinical and radiological correlates. Results Sixty-four year-old woman with a history of parotid gland ACC presents 15 years later with facial nerve paralysis. Work-up, including imaging and fine-needle aspiration biopsy, confirmed recurrent ACC with erosion of the Fallopian canal. She underwent surgical resection, which included radical parotidectomy and facial nerve sacrifice to negative frozen section margins halfway along the horizontal segment. Conclusions Acinic cell carcinoma, especially in the setting of recurrence, does have the potential for perineural invasion. With a high propensity for late recurrence, it is prudent to monitor patients with salivary gland ACC for at least a decade.
