• Cytomegalovirus (CMV) is the most important infective agent among kidney transplant receptors. CMV infeccion can sometimes develop a CMV syndrome (fever, leucopenia, trombopenia) or CMV disease (clinical afection and organ disfunction). It has been described risk situations frequently related to CMV infection-disease. So, there are necessary easy-quick diagnostic techniques with a high predictive value, and a rational use of antiviral drugs. Nowadays, the main test for viral charge monitorization are Agpp65 and DNA in serum, and Ganciclovir or Valganciclovir for profilaxis and treatment. All these considerations have lead to an important decrease of CMV disease. In this review, we propose different recomendations based in the most recent studies. El citomegalovirus (CMV) es el agente infeccioso más importante entre los receptores de trasplante renal. La infección por CMV en ocasiones lleva a síndrome por CMV (fiebre, leucopenia, trombocitopenia) o enfermedad por CMV (clínica síndrómica acompañada de disfunción orgánica). Se han descrito situaciones de riesgo frecuentemente relacionadas con la infección-enfermedad por CMV. Por tanto se hacen necesarias técnicas diagnosticas rápidas, con alto valor predictivo y fácilmente realizables, así como el uso racional y adecuado de tratamientos antivirales. Hoy son de elección la monitorización de la carga viral (Agpp65 CMV o ADN en sangre) para el diagnóstico y Gancilclovir o Valganciclovir para profilaxis o tratamiento. Estas medidas han conseguido una reducción importante e enfermedad por CMV. Proponemos unas pautas de actuación en base a lo descrito en la literatura.
