• Coexistence of MAC disease and lung cancer in the same nodular lesion is rare and often difficult to diagnose preoperatively. A solitary pulmonary nodule (SPN) with a thin-walled cavity in the right upper lobe of the lung was detected on a chest CT scan in a 59-year-old man. Repeated chest CT scans showed a growing tendency and wall thickening of the cavitary lesion. Right upper lobectomy was performed 43 months later. Histologically, the nodule was diagnosed as a MAC-infected granuloma with caseous necrosis complicated by small-sized adenocarcinoma (pT1aN1M0; Stage II A). An SPN with a cavity requires careful observation, and diagnosis should be confirmed by surgery if this rare condition is suspected
