Thrombectomy and clot analysis‐guided anti‐coagulant/platelet therapy for acute ischemic stroke in a pregnancy complicated with primary antiphospholipid syndrome: A case report

A 35‐year‐old woman with antiphospholipid syndrome, with 29‐week pregnancy, developed left hemiparesis. Diffusion‐weighted imaging showed a low Alberta Stroke Program Early Computed Tomography (ASPECT) score. The right internal carotid artery occlusion was recanalized by mechanical thrombectomy (MT) and a pathologically white thrombus was confirmed. Clot analysis‐guided anti‐coagulant/platelet therapy was continued, and the patient delivered at 38 weeks and was discharged with a modified Rankin Scale score of 1. We conclude that MT should be considered, even in pregnancy with antiphospholipid syndrome and low ASPECT scores.
