Squamous cell carcinoma (SCC) represents over 90% of oral malignant neoplasms, primarily affecting the tongues of men over 50 years of age. In the case of a 78-year-old male patient, who was both an alcoholic and a smoker, the initial presentation was a painless lesion in the anterior gingival region. Over the course of a year, this lesion evolved into a painful, enlarged area that extended from the midline to the right mandibular body. An incisional biopsy was performed with a suspicion of malignancy. Histological analysis revealed the presence of pleomorphic epithelial cells forming islands and nests, infiltrating the underlying connective tissue. Additionally, there were signs of cellular and nuclear pleomorphism, areas of keratinization with the formation of keratin pearls, and a moderate chronic inflammatory infiltrate. The diagnosis was established as well-differentiated squamous cell carcinoma. Subsequently, the patient was referred to the oncology service and has commenced radiotherapy treatment.
