Les complications des sinusites sont essentiellement secondaires aux sinusites ethmoïdales et frontales. Elles sont favorisées par l’immunodépression et par des anomalies anatomiques faisant communiquer l’endocrâne ou l’orbite aux cavités sinusiennes. La gravité tient au risque d’extension de l’infection à l’endocrâne et aux cavités orbitaires. Le diagnostic est à évoquer systématiquement devant une symptomatologie résistante au traitement et l’apparition de signes orbitaires ou neurologiques. Le scanner et l’IRM contribuent à l’orientation diagnostique et à l’évaluation de l’extension lésionnelle. Le traitement repose sur une antibiothérapie prolongée et sur une prise en charge chirurgicale dans le cadre d’une prise en charge pluridisciplinaire associant l’ORL, l’ophtalmologiste, le neurochirurgien et le réanimateur.
The complications of sinusitis are essentially secondary to ethmoidal and frontal sinusitis, occurring in patients weakened, particularly in case of immunodeficiency or anatomical defects. The gravity is due to the risk of spreading infection in intracranial tissues and orbital cavity. The diagnosis is always to discuss any symptomatology resistant to treatment and the appearance of orbital or neurological signs. The scanner and MRI contribute greatly to the orientation and diagnostic evaluation of the extent of the lesions. Treatment is based on prolonged antibiotic therapy and surgery and requires a multidisciplinary approach involving ENT, ophthalmologist, neurosurgeon and anesthesiologist resuscitator.
